What are the notary requirements for Connecticut?

If a seal is used it must include name exactly as on certificate of appointment, 'Notary Public' and 'Connecticut', plus 'My commission expires (date)' (the expiration may instead be on a separate stamp). No size/shape rule. Without a seal, 'Notary Public' and expiration must be typed/printed near the signature.
